Nata Mendlovic MT(ASCP), MBAChief Operating Officer, JFSA Alyson’s Place Medical Clinic
Nata Mendlovic, MT (ASCP), MBA is the Chief Operating Officer of Alyson’s Place Medical Clinic at the Jewish Family Service Association of Cleveland. Nata earned both her B.S. in Biology and an MBA from CSU, in addition to completing a Medical Technology program from CWRU/UHC. Nata has devoted most of her career helping people improve their quality of life. After spending over a decade in direct patient care of oncology/hematology patients at University Hospitals of Cleveland, she made it her mission to help individuals prevent major ailments rather than treat potential serious and life-threatening diseases. As the COO of Alyson’s Place, Nata focuses on expansion of services and partnerships with agencies and educational institutions, such as CWRU, throughout the community. She is leading JFSA Alzheimer’s Disease & Dementia Program including the new CMS GUIDE Initiative Model. The program’s goal is to enhance dementia public awareness, engagement, filling existing service gaps and expanding interdisciplinary supports for individuals living with Alzheimer’s Disease and related dementias and their caregivers throughout the progression of the disease.

Elizabeth O’Toole, MD
Geriatric Medicine, Alzheimer’s Disease & Dementia Consultant, JFSA
Dr. O’Toole graduated from CWRU School of Medicine and completed a residency in Internal Medicine with fellowships in General Internal Medicine – clinical ethics and Geriatric Medicine at University Hospitals of Cleveland/ Cleveland VAMC. She is a Professor of Medicine and Bioethics at CWRU School of Medicine.
Dr. O’Toole has over 30 years of experience in medical practice focusing on geriatric medicine and palliative care with an overall goal to provide patients and families with individualized care, education, and support. She has practiced at University Hospitals of Cleveland, the Cleveland VAMC, and most recently at MetroHealth Medical Center where she started and directed their Division of Palliative Care and initiated the Hospice and Palliative Medicine Fellowship Program at CWRU-MetroHealth. She also has worked and directed multiple externally funded research and educational programs. In addition, Dr. O’Toole has been involved in education and care through many community-based programs and has been a geriatric consultant for JFSA Cleveland for several years.
Resources to Help the Client and Caregiver Through the Dementia Journey
Nicole Hale, LMSW, CDPJFSA Care Navigator & Dementia Care Specialist
Nicole joined JFSA in 2018 and became a part of the Alzheimer’s Disease & Dementia Support Program team in 2021. Nicole earned her Bachelor of Arts from Malone University and her Master of Social Work from Cleveland State University. She is a Licensed Master Social Worker (LMSW) and Certified Dementia Practitioner (CDP). Nicole has a diverse range of experiences having worked at Summit County Probate Court, Summa Akron City Hospital, and as an English as a Second Language (ESL) teacher in Panamá. She completed her MSW field practicum at Cleveland Clinic’s Center for Lesbian, Gay, Bisexual and Transgender (LGBTQ+) Care. Nicole is passionate about enhancing the quality of life of older adults, a group she has engaged with from a young age. She enjoys building a relationship with her clients and their caregivers and helping to connect them with the resources and support they need in order to thrive.
Cassie Fazekas, LISW-S, MSSA, CDP JFSA Dementia Care Coordinator
Cassie earned her bachelor’s degree in social work at Ohio University then went on to get her Master of Social Science Administration with a Certificate in Gerontology at Case Western Reserve University’s Mandel School of Applied Social Sciences. Cassie also has earned a certificate as a Certified Dementia Practitioner. Cassie has dedicated her career to working with the older adult population. Cassie has a variety of experience from case management, long term care facilities, hospice and counseling. As a Dementia Care Coordinator, Cassie focuses on working with clients and/or caregivers to help access resources in the community, provide education and guidance and improve quality of life.
Hannah Doles, MGS, CMC, CDPManager of Care Navigation
Hannah joined the agency in 2018 and worked extensively with the Alzheimer’s Disease and Dementia Support Program for several years before transitioning into a management role supervising Care Navigators associated with the local older adult population, Holocaust survivors, and dementia program clients within the Older Adult Services Department.Hannah has a Bachelor of Science degree and Master of Gerontological Studies degree from Miami University. She has experience with working with caregivers through an employee assistance program and was a Guardianship Program Visitor with Butler County Probate Court. She is a Care Manager Certified and Certified Dementia Practitioner. Hannah’s goal is to serve local older adults and their families to ensure the best quality of life possible.
