Improved Care Coordination
Our medical staff works with JFSA case managers and social workers as needed to make sure you receive coordinated treatment. In addition, because of JFSA’s special collaboration with MetroHealth System, we can make direct referrals for specialty health care services. Patients and family members gain peace of mind because of the improved care coordination that results when patients use Alyson’s Place for their primary care needs.

Home Visits Available
If you are unable to visit the geriatric memory care clinic or receive your vaccinations at our office, we can visit you right in your own home! We can come to you when and where you need us, so you get the right care at the right time.
