professional medical care with the benefits of
home-based recovery, supporting both
physical health and emotional well-being.
Professional Medical Care at Home
Skilled home health services are provided by licensed healthcare professionals, such as nurses, physical therapists, occupational therapists, and speech therapists. This allows individuals to receive complex medical care in the comfort of their own homes, which can aid in recovery and management of conditions without the need for hospital stays. Services are typically delivered after a change in medical condition, including after a hospitalization, fall, or discharge from a skilled nursing facility. These services are goal-oriented and rehabilitative in nature.
Nursing
Registered nurses work in partnership with a client’s physician to develop a plan of care that includes an assessment, treatment visits and instruction for ongoing care. Typical situations that involve skilled nursing services include chronic disease management, post-surgical healing, wound care management, mobility and fall prevention, management of diabetes, dementia, and prescription medication education.
Physical, Occupational, & Speech Therapy
Therapies including physical, occupational and speech therapy are rehabilitative and goal-oriented treatments. The physical therapist provides guidance for large body movements, therapeutic exercise and instruction to improve mobility/ability and quality of life. The occupational therapist works with client’s to address their fine motor skills and daily living tasks. The speech therapist works with individuals to treat speech, language, social communication, cognitive communication and swallowing disorders that are often present as people age.
Medical Social Workers
Medical social workers provide counseling and referral to community resources for individuals adjusting to life with an illness or injury. They work closely with the medical team to develop and implement comprehensive care plans that address both medical and social needs. This coordination can involve arranging for home health care, follow-up appointments, or transitions to other care facilities.
Services are paid by:
Medicare, Medicare Advantage, the VA or Medicaid.
Subsidies are available for Holocaust Survivors through the Claims Conference and Jewish Federation of Cleveland.
