A unique program designed to support family members and caregivers dealing with a loved-one’s Alzheimer’s or other dementia-related disorder.

If a loved-one is showing signs of cognitive difficulties and you suspect the cause may be dementia, Alzheimer’s disease or another related disorder, JFSA can help you identify the services your loved-one needs as well as provide you with the education and support you need as a caregiver. 

Cognitive Screening and Assessment 

We begin with a thorough panel of evaluations performed by our care navigators, psychologist, and geriatricians. Our professional team ensures that your loved-one’s health, lifestyle, and needs are addressed. 

Coordinated Care 

Our team acts as a bridge among medical professionals, social service providers, and other essential services. We ensure an approach to care that encompasses not just the physical, but also the emotional and social needs of your loved one. Communication with families is paramount, providing not just updates, but also education and reassurance, ensuring peace of mind for you and your family. 

Tailored Information and Referrals 

Based on the unique needs of your loved one, we connect you with a variety of community services. These range from home care options to educational resources on brain health and dementia, companion care, and adult day programs. We also assist in identifying  financial options and legal pathways to secure the care and support your loved one deserves, safeguarding their rights and your peace of mind. 

ADDITIONAL SUPPORT AND RESOURCES

JFSA Memory Cafe 

Clients and caregivers enrolled in the dementia program can attend a monthly session where they can enjoy a range of fun and engaging social activities, and enjoy other appropriate activities. It is a way to enjoy activities that are providing them a break from the normal routine.

Brain Health & Wellness Speaker Series

The Brain Health & Wellness Speaker Series is a free online monthly live speaker series highlighting various aspects of brain health and what people need to know to keep their brain healthy at any age. Learn more and register

Fees:
Screening and assessments, referrals, Care Navigator, consultation with a geriatrician are provided free of charge. Some referred services may have fees associated with them.
