Care navigation helps families address the practical aspects of aging as well as the emotional and psychological challenges.
ABOUT CARE NAVIGATION
Care navigation is not just a service; it’s a lifeline for older adults and their families. Our dedicated care navigators partner with you and your loved ones to understand available resources, care options, and benefits, ensuring a smoother path to well-being and dignity in aging.
Establish and Implement Care Plan
We start by recognizing your loved one’s abilities, necessities, worries, and preferences, laying the foundation for a care plan that truly reflects their individuality and promotes their well-being and independence. By setting clear, achievable goals, we develop and execute a customized plan of care that addresses your loved one’s unique circumstances.
Connect to Government Benefits
Navigating the web of government assistance can be daunting. We simplify this process, ensuring your loved one receives the benefits to which they are entitled.
Coordinate Health and Support Services
From scheduling doctor appointments to organizing caregiver services, we ensure all aspects of your loved one’s care are seamlessly managed, relieving you of the logistical burdens and giving you more quality time to spend with your loved one.
Counseling
Our counselors provide older adults with support and strategies to navigate the emotional and psychological challenges that often accompany aging, including changes in independence and health, coping with loss, overcoming isolation and addressing family dynamics.
